C.A.R.E. provides case management services via two distinct levels of case management. The first level, Social Services Case Management, addresses the general psychosocial issues and needs of the client. These services are provided by one generalist case manager. The second level, Nursing Case Management, addresses the nursing, medical, and psychosocial issues and needs of the client. These services are provided by a registered nurse and a master’s level social worker. A client is assigned to one of the two levels of case management according to medical and/or psychosocial needs that have been identified by their medical provider.
Case managers are responsible for developing, implementing, monitoring, and evaluating an agreed upon individualized plan of care for each client. Advocacy is the primary role of case managers, and they do this by educating clients about resources and by referring them to appropriate community agencies as needed. Through intervention and education, case managers seek to help each client maintain the highest degree of independence or autonomy as possible.
